  • If you depend on a wheelchair for mobility, a proper fit is essential. With a doctor’s prescription, and following your insurance guidelines, we’ll arrange for a personalized Wheelchair/Powerchair Clinic.

At this one-on-one clinic you can expect:

Getting a new Wheelchair:
The Process from Beginning to End


At Angel Leaf Rehab & Consulting, evaluations will range from 60 to 90 minutes and may require a follow-up appointment.


Come to your appointment with :

·    Your current wheelchair if possible

·    Caregiver or family member as appropriate


During your visit our experts will evaluate and document the medical need for the equipment. After this, a doctor will review and approve the request. The vendor will submit the paperwork to the insurance company for approval. Once the paperwork is approved, the vendor will order the equipment and schedule a date for delivery to your home.
